Hello, I had a short experience within an SSIAD, as an intern, but I can describe the work of the psychologist. In addition, I met the patients alone too.
The bulk of the work consisted of providing home support to isolated elderly people. What was difficult was being able to do a real follow-up because these people often forgot who we were (due to Alzheimer's), even if we had already come to see them several times.
It was therefore imperative to call people before coming to see them, to remind them of the appointment.
The care team sent the psychologist cards on which they put which patient to see and why. Then the psychologist managed her appointments alone, and was very independent on that side. For families, the same, the psychologist could see them if they requested it, sometimes we could meet them "by chance" by going to the patient.
It was mostly about supporting older people who were feeling lonely, bored, and realizing their growing dependency. For families, support also to be able to manage the fact of having a sick relative. A whole question of acceptance of the disease.
I know that in some SSIADs, there are psychologists who set up discussion groups for family carers as well, which can be a good idea.
